Who we are
The Council’s Adult Locality Mental Health Team (ALMHT) aims to prevent deterioration in the mental health of Hounslow residents, increase the wellbeing of communities and support the recovery of residents with long-term mental illness.
The team is made up of mental health social care professionals who undertake individual casework aiming to meet the care and support needs, reduce isolation, promote independence and mental wellbeing.
What we do
The ALMHT works alongside health colleagues in Primary and Secondary Care as well as other stakeholders to link individuals with community support that maintains mental wellness, prevents or delays the development of social care needs that require a formal social support intervention. The ALMHT also is closely aligned to the Hounslow MINT (Mental Health Integrated Network Teams) provided by West London Mental Health NHS Trust.
