ALMHT is a team of social workers and assistants, committed to supporting individuals with mental health needs in the community.
We work closely with a range of partner agencies, including GPs, Mental Health Integrated Network Teams (MINT), Hounslow Crisis, Assessment and Treatment Team (HCATT), West London NHS Trust Single Point of Access (SPA), Housing, Police, and other voluntary agencies. Together, we strive to improve the mental wellbeing and promote the recovery of Hounslow residents.
Safeguarding Individuals
We have a statutory duty to safeguard individuals with mental illnesses. We achieve this by devising personalized protection plans in collaboration with our partner agencies. These plans are tailored to each individual’s needs, ensuring they are protected from any abuse and neglect while receiving the best possible care and support.
Early Intervention and Prevention
ALMHT aims to prevent and delay the development of needs through early intervention. We provide information and advice, signposting individuals to appropriate agencies when necessary. This proactive approach helps residents to address potential issues before they become significant problems.
Mental Capacity Assessment
For those who may struggle to make decisions about their care and support needs, we carry out mental capacity assessments in accordance with the Mental Capacity Act 2005. This helps to ensure that everyone receives the support they need, regardless of their ability to make decisions.
Support for Carers
We recognize the vital role that carers play in supporting individuals with mental illnesses. We assess the needs of carers, providing them with a personalised care plan to support their own mental wellbeing.
