Prevention and Care Management (Locality Teams)
As part of the ongoing commitment to improve services for the residents and to meet the requirements of the Care Act 2014, we are focussing on prevention and well-being. The London Borough of Hounslow has restructured its locality teams to align their functions with five GP locality areas within Hounslow.
You should hold a Social Work qualification – CQSW, DIPSW or BA in Social Work.
You will also need to demonstrate experience of working with the specific client group.
