Registering a death
From 9 September, all deaths must be scrutinised by the Medical Examiner (ME) before it can be registered.
The doctor (attending practitioner - AP) will discuss the cause of death with the ME before completing the Medical Certificate of Cause of Death (MCCD).
The MCCD may take several days to be issued as the doctor certifying the death will need to follow certain procedures before completing the paperwork.
All MCCDs will be sent from the ME office regardless where the death occured via email to the register office registeroffice@hounslow.gov.uk.
Registering a death in the London Borough of Hounslow
From 9 September, the timeframe in which a death must be registered will change. A death that has occurred in the London Borough of Hounslow must be registered within five days of the registrar receiving the MCCD or coroner's paperwork, not five days from the date of death.
Where the death has been referred to the coroner, it cannot be registered until the registrar has received written authority from the coroner.
All death registrations are by appointment only
Who can register the death:
- a relative of the the person who has died
- partner of the deceased (if the two of them were living as partners in an enduring relationship at the time of the deceased person’s death).
- a person present at the death
- the person arranging the funeral
- the manager of the home or hospital
- a person appointed by and acting on behalf of the deceased person’s family (such as a solicitor)
Upon completion of the registration, you will receive a certificate for burial or cremation, which is known as the Green Form (unless the coroner has already issued an order for burial or a certificate for cremation). This should be either taken to the funeral director so that the funeral can take place, or at your request, the registrar can email it directly to the funeral director.
There is no basis in law for the mortuary or those caring for the person who has died, to require sight of the green Certificate for Burial or Cremation in order to allow them to be collected.
The certificate is issued to allow the funeral to go ahead and for no other purpose.
Moving a person who has died for a funeral abroad
You need permission from a coroner to move a person who has died for a funeral abroad. Apply at least 4 days before you want them to be moved.
What to do if someone dies abroad?
You must register a death with the local authorities in the country where the person died.
