Change of details or loss of personal license
If you have lost your Personal Licence or the details on it such as your name or address have changed, you must contact us for a replacement.
Fees
The fee for a replacement Personal Licence is £10.50 payable AFTER we have received the relevant forms from you (below).

Change of details
If you are applying to change the details, both the A4 paper and plastic card licences must be returned along with the application form. If you are unable to return either of these licences then you must state a reason why. If you are only returning the forms, you may return them by email to licensing@hounslow.gov.uk
Loss of licence
Please return the form via email to licensing@hounslow.gov.uk
Processing and timescales
Applications will be processed within 14 days.
