Please ensure that you have your payment card details to hand and your Fixed Penalty Notice when starting this form.

Notes:

• The amount payable and the reference number is on the Fixed Penalty Notice you received Part A
• The format of the reference number looks like SWS/XXXXXX - a sample reference number looks like SWS/928345
• For multiple penalty notices, you will need to make individual submissions.
